Qualifications

• Must be a graduate of a nursing program approved by ACEN/CCNE
• RN with active, unrestricted nursing license in US state or territory
• Eligible for temporary appointment with GNT path until licensure complete
• State licensure upon completion of bridge program is required
• References VA qualification standards for nurse roles

Responsibilities

• Provide telephone triage and RN assessment per protocol
• Deliver patient education and case management across care continuum
• Coordinate care with multiple disciplines, services and community agencies
• Support transitions to ED or inpatient as needed
• Provide coverage for other clinics during clinical demand
