Job summary

Diversity Group is seeking an RN Care Manager to join our outpatient PACT team in a primary care setting. The RNCM will coordinate care for a defined patient panel, focusing on health promotion, chronic disease management and transitions of care.

The role requires strong care coordination skills, knowledge of VA/DoD guidelines, and the ability to work across multidisciplinary teams to support veteran-centered care.

Qualifications

• Graduate nurse with current US state licensure.
• Proficient with VA/DoD clinical practice guidelines.
• Experience in outpatient/PACT or patient-centered medical home model.
• Strong triage, critical-thinking and clinical assessment skills.
• Ability to prioritize workload and manage patient panels independently.

Responsibilities

• Coordinate care for a designated patient panel emphasizing health promotion and chronic care.
• Support transitions of care and preventive services using multiple modalities (in-person, phone, secure messaging, telehealth).
• Collaborate with PACT core team to improve access, continuity of care and quality outcomes.
• Utilize population health tools to identify rising/high-risk patients and coordinate referrals.
• Provide health coaching, motivational interviewing and chronic disease management per VA guidelines.
