Qualifications

• Nursing diploma (associate’s or bachelor’s degree in nursing).
• Active RN license in the applicable state and in good standing.
• One year of clinical post-acute care experience preferred.
• Experience with case management, utilization review, and discharge planning preferred.
• CCM or RN-BC certification preferred.

Responsibilities

• Coordinate patient care and services across care settings.
