Job summary

The LPN Unit Care Coordinator at Life Care Centers of America in Post Falls, Idaho supervises and coordinates patient care across the unit, working with patients, families, and nursing staff to ensure compliance with laws and Life Care standards.

Qualifications

• Attended an accredited LPN program.
• Active LPN/LVN license in good standing.
• CPR certification at hire or during orientation.

Responsibilities

• Direct daily functions of unit nurses and CNAs.
• Chart appropriately, accurately, and timely.
• Provide and coordinate patient care to maximize well-being.
• Administer medications as ordered by physician.
• Lead response to emergencies on the unit.
• Coordinate care plans with the RN.
• Maintain professional relationships and confidentiality.
• Follow Life Care policies and code of conduct.
