Job summary

Community Health Network in Indiana seeks a Care Manager RN for Post-Acute Care IHCI to guide transitions across care levels, coordinating assessment, planning, implementation and evaluation of patient needs. The role aims to improve quality of care and cost efficiency through integrated case management, utilization review and discharge planning.

You will collaborate with SNF and ACO teams to tailor care plans.

Qualifications

• 2 year / associate degree in nursing (required).
• Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) (required).
• 3+ years: nursing experience, preferably in Population Health management, Case Management, or Post-Acute Care. (required).
• Preferred Certifications: CCM, ACM, or equivalent certification.

Responsibilities

• Support the healthcare team towards a smooth transition between levels of care.
• Provide comprehensive assessment, planning, implementation, and evaluation of individual patient needs.
• Enhance quality of patient management and satisfaction; promote continuity of care and cost effectiveness.
• Manage post-acute services for patients across SNFs; communicate length of stay and coordinate care plans across facilities within the ACO.
