Job summary

LCMC Health is seeking a Care Manager to serve as the hub of patient case management and the care plan. You will develop individualized plans, coordinate across the care team, and guide transitions to achieve the best patient outcomes.

You will make outbound calls, identify treatment gaps, coordinate referrals, and ensure safe discharges. Documentation in EPIC and collaboration with physicians will be essential to meet quality and cost goals. Louisiana RN licensure required.

Qualifications

• RN licensed to practice in Louisiana with an active license.
• 2 years of professional nursing or care management experience.
• Experience coordinating care across the continuum and educating patients/families.

Responsibilities

• Develop individualized care plans considering patient and caregiver goals.
• Make outbound calls to assess health status and gaps in treatment.
• Coordinate care and referrals, ensure safe discharges and medication reconciliation.
• Triage symptom-based calls and enroll patients in home monitoring programs.
• Document workflows in EPIC and provide patient education for self-management.
• Collaborate with care team, leadership, and physicians to meet quality and cost objectives.
