Job summary

LCMC Health is seeking a Care Manager to serve as the central hub for patient case management and care plans. You will collaborate with the care team, implement plans, and coordinate transitions to ensure quality and safe outcomes.

Responsibilities include developing individualized plans, coordinating care, educating patients, and documenting workflows in EPIC. A Louisiana-licensed RN with at least 2 years of nursing/care management experience is required.

Qualifications

• Current nursing license required to practice.
• Minimum 2 years of professional nursing or care management experience.

Responsibilities

• Develop individualized care plans and goals with patient and family.
• Make outbound calls to assess health status and follow up.
• Identify gaps or barriers in treatment plans and coordinate care across providers.
• Coordinate care and referrals to outside sources.
• Ensure discharge planning and medication reconciliation where applicable.
• Triage symptom-based calls per approved protocols.
• Enroll patients in home monitoring programs and manage alerts.
• Educate patients to support self-management and disease understanding.
• Document workflows in EPIC for tracking and quality.
• Collaborate with care team and physicians to meet quality and cost objectives.
