• Drug screen test with negative result.

Responsibilities

• Observe and assess referred patients to identify care needs.
• Monitor care/case managed patients with health professionals.
• Lead patients through their care plans and set achievable goals.
• Educate patients on procedures, meds, diets, and care instructions.
• Coordinate with healthcare team to plan and evaluate care.
• Review records for completeness and regulatory compliance.
• Assist in quality improvement and safety initiatives.
