Regions Hospital is seeking a Hospital Care Management Supervisor to lead the department's daily operations and support the manager with planning and coordination of services. You will provide leadership, guidance, and consultation to program staff while promoting collaborative patient care.

The role includes mentoring staff, conducting psychosocial assessments, developing care plans, and referring patients and families as needed, using a range of clinical approaches to improve outcomes.

Qualifications

• Bachelor's Degree in Nursing and/OR Master's in Social Work.
• 5 years' experience in your profession.

Responsibilities

• Mentoring and coaching staff, supporting department leaders, and providing constructive performance feedback.
• Conducting clinical and psychosocial assessments, developing care plans, and making appropriate referrals for patients and families.
