Qualifications

  • Requires an Associate's Degree in Nursing or nursing diploma upon hire.
  • California RN license at time of hire.
  • Minimum 2 years of acute care experience in Medical Surgical or Inpatient settings.
  • At least 1 year in care management or utilization review, or completion of TIPS/Case Management Orientation.

Responsibilities

  • Carry out effective case management for identified patients across the continuum of care.
  • Plan, coordinate, negotiate, procure and manage care for complex patients and families.
  • Communicate daily with medical staff, hospital personnel and external resources to ensure appropriate resource utilization.
