Job summary

Providence Redwood Memorial Hospital is seeking a per diem RN Care Manager for an 8-hour day shift. You will carry out comprehensive case management for identified patients across the continuum of care, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families.

Daily collaboration with medical staff and external resources ensures appropriate utilization of resources and aligns with clinical and financial standards.

Qualifications

• ASN or nursing diploma required at hire.
• California RN license required on hire.
• 2 years acute care experience in Medical/Surgical or inpatient settings.
• 1 year in care management or utilization review, or completion of related program.

Responsibilities

• Carry out effective case management for identified patients across the continuum of care.
• Plan, coordinate, negotiate, procure and manage care for complex patients and families.
• Communicate daily with medical staff, hospital personnel and external resources to ensure appropriate resource utilization.
• Monitor and document care plans and patient outcomes.
