Requires California RN license and ADN; 2 years of acute care experience and 1 year in care management or utilization review. TIPS or Case Management Orientation may be completed on hire.

Qualifications

• ADN/ASN required at hire.
• California RN license required at hire.
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year in care management or utilization review; TIPS or Case Management Orientation program may be completed upon hire.

Responsibilities

• Assess and plan care for at-risk patients.
• Coordinate care and manage resource utilization.
• Discharge planning and documentation of interventions.
• Ensure regulatory compliance and patient advocacy.
