Job summary

NYU Langone Hospital—Suffolk is seeking an experienced Care Manager - Registered Nurse for full-time nights in the Care Management team. The RN Care Manager collaborates with the interdisciplinary team to implement the plan of care, transition strategies, and ensure desirable patient outcomes while optimizing length of stay and resource use.

The role emphasizes utilization management, payer communication, discharge planning, and advocacy for patients and families across care settings.

Qualifications

• Current NYS Registered Nursing License required.
• BSN/MSN preferred and Case Management certification preferred.
• Minimum 5 years of clinical experience and 3–5 years as hospital inpatient Case Manager.
• Knowledge of federal/state regulations on hospital reimbursement and discharge planning.

Responsibilities

• Manage utilization management and payer communications for authorization and payment.
• Perform admission review within 24 hours using InterQual Criteria.
• Coordinate with attending physicians to support medical necessity for admission.
• Lead transition planning and discharge planning from admission to discharge.
• Document reviews and outcomes in the electronic medical record.
• Escalate complex cases per department guidelines and coordinate with social work as needed.
• Ensure continuity of care across settings and advocate for patients and families.
