Qualifications

• Current NYS Registered Nursing License; BS/MS preferred.
• Case management certification preferred.
• Minimum 5 years of clinical experience and 3–5 years as a hospital case manager.
• Knowledge of federal/state hospital reimbursement and utilization review.
• Working knowledge of CDI and hospital software/documentation.

Responsibilities

• Utilization management with payers to secure authorization and payment.
• Admission review within 24 hours using InterQual criteria.
• Collaborate with physicians and care team to ensure appropriate level of care.
• Lead discharge planning and coordinate post-hospital care.
• Maintain thorough documentation in the electronic medical record.
• Communicate with patients and families regarding guidelines and plans.
