Job summary

Qualifications

Responsibilities

• Collaborate with the interdisciplinary team to implement the plan of care.
• Perform admission review within 24 hours of admission utilizing InterQual Criteria.
• Initiate contact with attending physician for additional information to support medical necessity.
• Suggest alternative level of care/treatment plans for patients not meeting medical necessity.
• Document reviews in Soft Med according to department procedure.
• Advocate for patients and families to obtain benefits from third-party payors.
• Lead and implement the transition planning process from admission to discharge.
• Complete discharge planning assessments for all patients within 24 hours of admission.
• Ensure continuity of care by acting as a liaison between the hospital and community resources.
• Monitor patients' plans of care and progress in relation to anticipated length of stay.
