Qualifications

• Highly developed verbal and written communication skills.
• Good organizational skills and ability to prioritize many competing time demands.
• Ability to communicate/collaborate with team members, payors and community agencies.
• Advanced problem solving skills.

Responsibilities

• Discharge planning: identify complex cases, coordinate with care manager to ensure timely discharge and linkage to post-hospital care.
• Crisis intervention/counseling: assess abuse cases, provide crisis intervention and refer to services as needed.
