Job summary

Roper St. Francis Healthcare is seeking an RN Care Manager to coordinate patient care and ensure safe transitions across the continuum.

You will assess needs, plan care with the team, and oversee implementation from admission to discharge, with a focus on preventing readmissions and aligning resources for optimal outcomes. The role requires an active RN license, BLS, and 1 year of clinical experience; 3 years in acute care is preferred.

Qualifications

• RN license in the state where working or compact (required).
• BLS Basic Life Support – American Heart Association (required).
• 1 year of experience in clinical setting (required).
• 3 years of experience in an acute care clinical setting (preferred).
• Ambulatory or post-acute care coordination experience (preferred).

Responsibilities

• Identifies and prioritizes patients in need of care management services, planning with patient and team to maximize outcomes.
• Completes documentation reflecting needs and plans in the patient record and ensures clear handovers.
• Documents plan of care addressing post-hospital care needs and involves patient/family in planning.
• Supports discharge planning, care coordination, and readmission prevention initiatives.
• Collaborates with revenue cycle and patient access to ensure accurate care classifications and timely communications.
