Job summary

CommonSpirit Health is seeking an RN Care Coordinator to serve as a central figure in patient care, navigating the healthcare journey to achieve optimal outcomes and an exceptional patient experience.

You will assess, plan, and facilitate care across the continuum, advocate for patients, and collaborate with physicians, nursing, insurers, and post-acute providers to ensure timely, high-quality transitions and reduced readmissions.

Qualifications

• Graduate of an accredited school of nursing and minimum two years of acute hospital clinical experience.
• Masters in Case Management or Nursing field with at least one year of experience.
• Registered Nurse license in NE.
• Basic Life Support (CPR) certification within 90 days.

Responsibilities

• Completes and documents discharge planning assessments as identified by screening process.
• Develops a multidisciplinary discharge plan with patient, family, and post-acute providers.
• Oversees and evaluates implementation of discharge plan.
• Collaborates with multidisciplinary team to ensure appropriate inpatient resource utilization.
• Coordinates with post-acute services for timely transitions based on clinical needs.
• Identifies readmission risk and implements interventions for at least 30 days.
