Job summary

Virginia Mason Franciscan Health is seeking an RN Care Coordinator to centralize patient care and optimize transitions across the continuum. You will assess, plan, and advocate for patients while coordinating with physicians, nursing, insurers, and post-acute providers to ensure timely, high-quality care.

The role emphasizes strong clinical acumen, exceptional communication, and a strategic mindset to improve patient outcomes and experiences within our integrated health system.

Qualifications

• Graduate of an accredited nursing program.
• Minimum two years acute hospital clinical experience.
• WA Registered Nurse license.
• Basic Life Support (CPR) within 90 days of hire.

Responsibilities

• Completes and documents discharge planning assessments.
• Develops multidisciplinary discharge plans with input from patient and post-acute providers.
• Oversees and evaluates discharge plan implementation.
• Collaborates with care teams to ensure progression of care and appropriate inpatient resource use.
• Coordinates transitions to post-acute services based on clinical needs.
• Identifies readmission risk and implements mitigations for at least 30 days.
