Job summary

Care To Stay Home Inc. is seeking an Idaho-based RN Care Manager to lead and develop our care management team.

Reporting to the Executive Director, you will supervise two Assistant Care Managers and guide exceptional care for North Idaho seniors in their homes. Use assessment and leadership skills to build individualized care plans, strengthen caregiver competence, and support lasting client and family relationships.

Qualifications

• Active Registered Nurse (RN) license is required.
• Bachelor’s in Nursing and 5+ years’ geriatric/home-care experience preferred.
• Demonstrated staff supervision and leadership skills.
• Solid clinical assessment skills and delegation capabilities.
• Warm, professional communication that builds trust with families and field staff.

Responsibilities

• Complete in-home client assessments and develop individualized care plans.
• Supervise assistant care managers and provide ongoing caregiver coaching.
• Monitor condition changes and document interventions in the EMR.
• Coordinate with scheduling and external physicians to keep care seamless and client-centered.
• Maintain audit-ready client files and comply with home-care regulations.
• Provide nurse delegation services within state-specific qualifications.
