BoldAge PACE is seeking a Registered Nurse Care Manager to assess and coordinate the care needs of participants in clinic, center, and home settings. You will work with the interdisciplinary team to develop plans of care, delegate tasks, and ensure timely follow-up with providers and specialists.

You will provide nursing interventions, monitor outcomes, and educate participants and families on medications, self-care, and healthy lifestyles while maintaining regulatory compliance and quality care

Qualifications

• Bachelor of Science in Nursing preferred.
• RN license required (NJ).
• BLS required (within 90 days).
• 1 year of experience with frail or elderly population preferred.
• Experience in home/long-term care or managed care is preferred.

Responsabilités

• Provide high-quality clinical nursing care in clinic, center, and participants' homes.
• Collaborate with the IDT to develop and implement care plans.
• Assess participants' needs and coordinate home care services.
• Educate participants and families on self-care, medications, and safety.
• Participate in 24/7 on-call triage for participant needs.
• Ensure documentation in medical records and channel communications with providers.
