Lakeland Care, Inc. is seeking an RN Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities in Wisconsin.

The RNCM arranges services based on comprehensive assessments and member-centered plans, coordinating care across providers. As part of the IDT, you will conduct home visits, participate in care conferences, and ensure DHS contract compliance while safeguarding PHI under HIPAA.

Competenze

• Current License to practice as a Registered Nurse in Wisconsin.
• Bachelor’s degree in nursing strongly preferred.
• Two (2) or more years of skilled nursing experience preferred, ideally in gerontology/disabilities and/or home care.
• Ability to access members’ homes which are not required to comply with ADA regulations.
• Ability to lift up to 25lbs.
• Current driver’s license, acceptable driving record and proof of adequate insurance.

• Conduct comprehensive assessments of member outcomes, needs and risks and reassess as conditions change.
• Coordinate acute and primary care services, care transitions and follow-up care.
• Develop, monitor, and evaluate member-centered plans considering cost and effectiveness.
• Promote prevention and wellness within the care planning process.
• Collaborate with the Care Manager to implement risk mitigation strategies for health, safety, and independence.
• Participate in ongoing training to ensure compliance with regulations and policies.
• Create and maintain member records as required by DHS and policy.
• Engage in in-home visits and care conferences to assess and address health needs.
• Participate in 24-hour on-call rotation and team meetings.
• Provide nursing care per Nurse Practice Act and maintain PHI confidentiality.
