Job summary

• Requires three years of recent clinical experience in acute care, home health, or long-term care.
• Hired applications expected to obtain Case Management certification.

Responsibilities

• Reviews admissions and prioritizes patients; documents assessment and care plan in EMR.
• Participates in multidisciplinary team discussions when discharge barriers arise; coordinates family meetings as needed.
• Provides education on community resources and coordinates referrals to post-acute providers.
• Coordinates with units to monitor patient progression and adjust discharge plans; assists with estimating EDD.
• Collaborates with care team to problem-solve for complex cases and escalates high-risk situations as needed.
• Maintains system for monitoring admissions and discharge planning; acts as liaison with payers and regulatory bodies.
