Job summary

Qualifications

• Licensed to practice as a Registered Nurse in North Carolina or a compact state.
• Three years of recent clinical experience in an acute care setting.
• Basic Life Support (BLS) certification required.

Responsibilities

• Review admissions and prioritize patients; document assessment and plan of care in the EMR.
• Coordinate discharge planning with the multidisciplinary team to support progression of care.
• Educate patient and family about community resources and referrals to post-acute providers.
• Collaborate with care team to monitor high-risk cases and adjust discharge plans accordingly.
• Maintain documentation for third party payers and other agency requirements.
