Job summary

Molina Healthcare is seeking a registered nurse to provide care management and coordination across the continuum, collaborating with a multidisciplinary team to improve member outcomes and cost-effective care. The role requires comprehensive assessments, plan development with members and caregivers, and ongoing monitoring of progress.

You will conduct visits, educate members, facilitate ICT meetings, and address barriers to care while promoting integration of services across behavioral health,

Qualifications

• Active RN license in state of practice.
• 2+ years healthcare experience, ideally care management.
• Knowledge of EMR systems and HIPAA compliance.
• Strong communication and time-management skills.

Responsibilities

• Complete member assessments within regulated timelines and identify care-management candidates.
• Develop and implement care coordination plans with members and care networks.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and adjust care plans based on progress and barriers.
• Manage a ongoing member caseload and coordinate ICT meetings.
• Promote integration of behavioral health, LTSS, and community resources.
• Provide education and support using motivational interviewing.
• Address barriers to care and document interventions.
