Job summary

Endeavor Health is hiring an RN Care Manager in Glenview, IL to coordinate care and manage authorization processes. The role requires an IL nursing license, a BSN, and at least 2 years of clinical nursing experience with utilization management or chronic disease management preferred.

EMR experience and knowledge of InterQual/MCG criteria are beneficial. The position is full-time, with daytime hours, on-call responsibilities, and rotating weekend coverage.

Qualifications

• LICENSE: Current professional Nursing Licensure in IL required.
• EDUCATION: Bachelor's degree in Nursing; NLN accredited school preferred.
• CERTIFICATION: Case management certification beneficial; BLS/CPR certification.
• EXPERIENCE: Minimum 2 years clinical nursing; Utilization Management or chronic disease mgmt preferred.

Responsibilities

• Facilitates payor authorization processes and ensures accurate patient type.
• Coordinates patient care across the continuum to remove delays in care and reimbursement.
• Collaborates with the CDI team to reflect severity of illness and intensity of service.
• Maintains communication with physicians, nursing and multi-disciplinary team for timely progression.
• Identifies and resolves delays to discharge to optimize length of stay.
• Seeks multidisciplinary consultation to expedite care and discharge as needed.
• Monitors progress and ensures a high-quality, cost-effective plan of care.

Hourly Pay Range:

$41.86 - $64.88 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

• Position: RN Care Manager
• Location: Glenview, IL
• Full Time: 40 hours per week
• Hours: Monday-Friday, 8:30a-5:00p, on call, rotating weekends every 3-4 weeks
• Required: 1 winter and 1 summer holiday coverage

What you will need:

• License: Current professional Nursing Licensure in the state of IL required
• Education: Bachelor's degree required. Bachelor's degree in Nursing from a NLN accredited school of nursing is preferred.
• Certification: Clinical certification, such as case management certification, is beneficial. Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross).
• Experience: Minimum of 2 years clinical nursing experience required; Previous experience in Utilization Management, Managed Care or chronic disease management highly preferred. Experience with Electronic Medical Record (EMR) platform preferred. Knowledge of InterQual or MCG criteria is preferred.

What you will do:

• Facilitates payor authorization processes and ensures that the patient type assigned is accurate.
• Facilitates the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.
• Works with the CDI team to ensure that documentation accurately reflects severity of illness and intensity of service.
• Works collaboratively and maintains active communication with physicians, nursing and other members of the multi-disciplinary care team to effect timely, appropriate patient progression.
• Addresses/resolves system problems impeding diagnostic or treatment progress. Proactively identifies and resolves delays and obstacles to discharge so that length of stay is optimized.
• Seeks consultation from appropriate disciplines/departments as required to expedite care and facilitate discharge.
• Monitors the patient?s progress, intervening as necessary and appropriate to ensure that the plan of care and services provided are patient focused, high quality, efficient, and cost effective.
