Job summary

Endeavor Health is seeking an RN Care Manager in Glenview, IL for a full-time, 40-hour week. Typical hours are Monday–Friday, 8:30 a.m. to 5:00 p.m., with on-call duties and rotating weekends every 3–4 weeks.

Requires IL nurse licensure, a BSN, and at least 2 years of clinical nursing experience. Experience in utilization management or chronic disease care is strongly preferred, along with EMR familiarity and knowledge of accountability criteria such as InterQual/MCG.

Qualifications

• IL nursing licensure required
• Bachelor's degree in Nursing required
• NLN-accredited BSN preferred
• Clinical certification in case management beneficial
• BLS/CPR certification required
• Minimum 2 years clinical nursing experience
• Experience with UM, chronic disease management preferred
• EMR platform experience preferred
• Knowledge of InterQual/MCG criteria preferred

Responsibilities

• Facilitates payor authorization processes and ensures correct patient type
• Manages patient care across continuum to remove barriers to timely care and reimbursement
• Ensures documentation reflects severity of illness and intensity of service
• Maintains communication with physicians, nursing, and care team for patient progression
• Addresses system problems impeding diagnostic or treatment progress
• Identifies and resolves delays to discharge to optimize length of stay
• Seeks consultations to expedite care and discharge
• Monitors progress to ensure high quality, cost-effective care

Hourly Pay Range: $41.86 - $64.88 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights

Position: RN Care Manager Location: Glenview, IL Full Time: 40 hours per week Hours: Monday-Friday, 8:30a-5:00p, on call, rotating weekends every 3-4 weeks Required: 1 winter and 1 summer holiday coverage

What you will need

• License: Current professional Nursing Licensure in the state of IL required
• Education: Bachelor's degree required
• Bachelor's degree in Nursing from a NLN accredited school of nursing is preferred
• Certification: Clinical certification, such as case management certification, is beneficial
• Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross)
• Experience: Minimum of 2 years clinical nursing experience required
• Previous experience in Utilization Management, Managed Care or chronic disease management highly preferred
• Experience with Electronic Medical Record (EMR) platform preferred
• Knowledge of InterQual or MCG criteria is preferred

What you will do

• Facilitates payor authorization processes and ensures that the patient type assigned is accurate.
• Facilitates the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.
• Works with the CDI team to ensure that documentation accurately reflects severity of illness and intensity of service.
• Works collaboratively and maintains active communication with physicians, nursing and other members of the multi-disciplinary care team to effect timely, appropriate patient progression.
• Addresses/resolves system problems impeding diagnostic or treatment progress.
• Proactively identifies and resolves delays and obstacles to discharge so that length of stay is optimized.
• Seeks consultation from appropriate disciplines/departments as required to expedite care and facilitate discharge.
• Monitors the patient’s progress, intervening as necessary and appropriate to ensure that the plan of care and services provided are patient focused, high quality, efficient, and cost effective.
