Job summary

The Covenant Health system is seeking a Registered Nurse Care Manager II at Fort Sanders Regional Medical Center. This PRN/OCC position supports patient care coordination, staff education, and clinical leadership on the nursing unit.

You will integrate evidenced based practice, develop care guidelines, participate in multidisciplinary rounds, and collaborate with physicians and care teams to ensure efficient transitions and quality outcomes for the designated patient population.

Qualifications

• Bachelor’s degree in Nursing or related field, or equivalent certification.
• Four years RN experience; at least three years in area of responsibility.
• Current Tennessee RN license and case management certifications (CCM/ACM/CPHQ) or related degree.

Responsibilities

• Assess patients and develop comprehensive care plans using the nursing process.
• Collaborate with physicians and health care team to plan and implement care.
• Educate staff and patients on medications, treatment plans, and discharge instructions.
• Participate in daily multidisciplinary rounds and quality improvement initiatives.
• Coordinate, organize and document care management activities and resources.
