Covenant Health is seeking a Registered Nurse Care Coordinator II in Quality Management. This full-time role integrates evidenced-based clinical practice, coordinates staff and patient education, and serves as a clinical resource for the health care team in East Tennessee.

The RN Care Coordinator II develops care coordination plans, promotes quality improvement initiatives, and collaborates with providers to ensure optimal patient outcomes.

Competenze

• Bachelor's degree in Nursing or related field.
• Current Tennessee RN License.
• Four (4) years of acute care nursing experience; minimum three (3) years in area of assigned responsibility.

Mansioni

• Coordinate care plans and education for staff and patients.
• Collaborate with the CDI and care teams to document and monitor care plans.
• Lead quality improvement initiatives and develop practice guidelines for the assigned population.
