Qualifications

• Current RN license in state of practice.
• BSN from an accredited institution; degree verification required; BSN must be obtained within four years of hire/promo.
• Demonstrated clinical nursing experience in an inpatient hospital setting; familiarity with hospital terminology.
• Understanding of disease management including treatment, LOS, barriers to delivery of care, and variation.

Responsibilities

• Assess patients using screening criteria and conduct comprehensive evaluations.
• Develop, adjust, and coordinate care plans addressing medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Educate healthcare teams and patients; advocate for patient rights and self-care.
• Collaborate with multidisciplinary teams to ensure timely, high-quality care.
• Lead initiatives to improve care transitions and policy adherence.
• Provide Goals of Care education for informed decision-making.
• Track metrics, analyze trends, and ensure regulatory compliance.
