AHF seeks an RN Medical Care Manager in North Miami Beach, FL to deliver coordinated nursing care within the Ryan White program. You will work with physicians, Care Management, and the Healthcare Center to ensure patients receive education, resources, and support for HIV management.

The role emphasizes patient education, care planning, adherence support, and collaboration across teams to meet program goals and improve outcomes for vulnerable populations.

Competenze

• RN with clinical nursing skills and knowledge of Ryan White Program.
• Ability to provide education and follow-up to clients.
• Collaboration with physicians, Care Management team and Healthcare Center.

Mansioni

• Conduct Needs Assessments and develop Care Plans.
• Provide ongoing education to clients about diagnoses and medications.
• Coordinate care with clinicians to ensure optimal outcomes and adherence.
• Perform retention calls to minimize gaps in care and retention.
• Assist with ACA enrollment for eligible clients.
