South Florida Community Care Network, LLC is hiring a Population Healthcare Management RN Case Manager for the Ryan White program in Sunrise, FL. The role supports an HIV care continuum with care plans, education, and coordinated interventions.

The position emphasizes multidisciplinary collaboration, patient education, and adherence to evidence-based guidelines to improve health outcomes and reduce costs.

Competenze

• Registered nurse licensure in Florida.
• Certified case manager or certified diabetes educator preferred.
• Minimum of five years of clinical experience or two years in case/disease management with HIV/AIDS care.
• Knowledge of Microsoft Office and internet software.

Mansioni

• Coordinate, educate, and provide expertise to clients living with HIV/AIDS across the continuum of care.
• Coordinate healthcare interventions to facilitate care at the lowest safe level and empower self-management.
• Work with Memorial Healthcare System’s Infectious Disease division as part of a multidisciplinary team.
• Participate in Multidisciplinary Case staffing and team huddles to plan care.
• Analyze clinical information to determine enrollment eligibility for Ryan White Disease Case Management.
• Review daily schedules, arrange face-to-face visits and education as needed.
• Provide outreach to clients with chronic conditions and preventive care needs.
• Develop individualized care plans with measurable goals and timelines.
• Collaborate to optimize adherence to medical plans and preventive care screenings.
• Update care plans quarterly and communicate with the team.
