Job summary

Variety Care Inc. is seeking an RN Care Manager to support high-risk patient populations and coordinate comprehensive care within the practice. The RN will assess physical, mental, and psychosocial needs, develop plans to prevent deterioration, and ensure effective care across the care team.

The role emphasizes behavioral strategies, patient engagement, and collaboration with providers to improve outcomes and reduce unnecessary ER usage. Strong communication and leadership are essential.

Qualifications

• Active RN license in good standing in Oklahoma.
• Excellent oral and written communication skills.
• Two years of experience in care management in a community-based setting.
• Bachelor of Science Degree in Nursing is preferred.

Responsibilities

• Manage Variety Care’s high-risk patient population.
• Conduct comprehensive assessment of patients’ physical, mental, and psychosocial needs.
• Develop care plans to prevent disease progression, improve outcomes, increase engagement, and reduce hospital/ER utilization.
• Reconcile patient medications and coordinate with the care team.
• Communicate with providers and optimize information flows among the office-based care team.
• Utilize behavioral strategies to promote self-management and healthy behaviors.
• Coordinate services across Behavioral Health, Social Services, Enrollment, Nutrition, Pharmacy, Optical and Dental.
• Lead a subset of the care management team and assist in prioritizing interventions.
