Job summary

AdventHealth in Tarpon Springs, FL is seeking a Registered Nurse to lead discharge planning and care coordination. The role involves multi-disciplinary rounds, educating patients and families, and coordinating resources for timely discharge.

The ideal candidate has RN licensure, 2+ years of hospital nursing experience, and experience in care management or utilization management. This PRN Day position offers opportunities for professional growth and community impact.

Qualifications

Associates degree in Nursing (ADN) required.
Bachelor’s degree in Nursing preferred.
2+ years medical/hospital nursing experience required.
Care management/utilization experience preferred.
RN license required; BLS/CCM/ACM preferred.

Responsibilities

Participates in multi-disciplinary rounds to review patient status, care progression and discharge plans.
Educates patients and families on emotional, social, and financial aspects of illness and discharge planning.
Assesses readmitted patients to identify reasons and needs for discharge.
Organizes and facilitates patient and family care conferences with the team.
Documents discharge plans and barriers to care.
