Job summary
AdventHealth in Tampa, FL is seeking an experienced Registered Nurse to lead discharge planning and care coordination across inpatient and observation units. You will work with a multidisciplinary team to ensure timely and safe transitions from hospital to post-acute care, while addressing social, financial and family needs.
Responsibilities include discharge planning evaluation, educating patients and families, and coordinating resources with payors.
Qualifications
- RN license required per Florida law.
- Associates degree in Nursing required; BSN preferred.
- Minimum 2+ medical/hospital nursing experience.
Responsibilities
- Participate in multidisciplinary rounds to review patient status and discharge plans.
- Educate patients and families on illness impact and mobilize resources for post-discharge needs.
- Assess readmitted patients and determine reasons for readmission.
- Organize and facilitate patient and family care conferences with the team.
- Document discharge planning evaluations and barrier notes for timely progression.
- Coordinate post-acute care authorization with payors when needed.
