Job summary

AdventHealth in Riverview, FL is seeking a Registered Nurse with discharge planning responsibilities to coordinate patient transitions from hospital to post-acute care. You will participate in multidisciplinary rounds, assess needs, and ensure timely discharge while communicating with families, payors, and care teams.

The role requires strong leadership, clinical knowledge, and the ability to work in a dynamic healthcare environment.

Qualifications

• Associates of Nursing required.
• Bachelors of Nursing preferred.
• RN licensure required.

Responsibilities

• Actively participates in multi-disciplinary rounds to review patient status, progression and discharge plans.
• Educates patients and families on illness impacts and resources; advocates for patient empowerment.
• Assess readmission reasons and organize care conferences with the team.
• Document discharge planning evaluations and communicate with payors for post-acute authorization.
• Review medical records to inform discharge planning and ensure timely care transitions.
• Develop discharge plans with contingency and post-acute care arrangements.
• Utilize standard work and technology to ensure complete records and accurate discharge reconciliation.
• Perform other duties as assigned.
