Job summary

AdventHealth in Florida is seeking a Registered Nurse for discharge planning to coordinate transitions of care across inpatient, observation, and emergency departments. You will lead discharge planning conferences, educate patients and families, and collaborate with payors for post-acute authorization.

The role emphasizes a holistic approach to discharge, community resources, and ensuring timely progression of care in a dynamic healthcare environment.

Qualifications

• RN license required.
• 2+ years in medical/hospital nursing.
• Care management/utilization management experience preferred.

Responsibilities

• Actively participates in multi-disciplinary rounds to review patient status, progression, and discharge plans.
• Educates patients and families on emotional, social, and financial impacts of illness and mobilizes resources for identified needs.
• Assess readmitted patients for reasons of readmission.
• Organize and facilitate patient and family care conferences with the multidisciplinary team.
• Document discharge planning evaluations, discharge plans, and barriers to progression of care.
• Assess wholistically for discharge planning needs across inpatient, observation, and ED settings.
• Review medical records and incorporate clinical, social, and financial factors into transition of care.
