Qualifications

• BSN from accredited nursing program; ADN acceptable if completed within 5 years of hire.
• Annual mandatory education requirements must be completed.
• Minimum of two years of relevant clinical experience.
• Current RN license; compact multi-state license; BLS preferred or required within 3 months of hire.
• Case Management certification and NCC certification preferred.

Responsibilities

• Provide age-appropriate nursing care per population-specific criteria.
• Assess patient needs and develop individualized care plans with the care team.
• Initiate discharge planning upon admission and coordinate referrals as needed.
• Collaborate with physicians and interdisciplinary teams to optimize patient outcomes.
• Document medical records accurately and timely; communicate results and progress.
• Participate in rounds, performance improvement activities, and committee work.
• Teach patients and families about complex conditions and discharge instructions.
