Qualifications

Responsibilities

• Facilitates coordination of a multidisciplinary plan of care to guide patient progress to optimal care transitions
• Performs evaluation of patient’s care and progress for the episode of care and/or across the continuum of care for the identified population
• Reviews patient medical records to determine justification of admission, treatment and continued stay criteria; determines and facilitates appropriate post-hospital discharge destination
• Identifies patient needs, capacity and preferences to determine what is required for a smooth and safe transition to discharge destination
• Actively involves patient, or individual acting on patient’s behalf, in planning for discharge

• Diploma Of Nursing Graduate of accredited professional nursing education program
• Registered Nurse: MN

Preferred

• Bachelors Degree in a healthcare field
• Experience as a Discharge Coordination and/or in Case Management
