Job summary

AdventHealth in Denver, CO is seeking a skilled Registered Nurse Care Manager to coordinate discharge planning and post-acute needs across the care continuum.

You will lead multidisciplinary rounds, assess patient and family needs, educate on resources, and ensure timely transitions. RN licensure and 2+ hospital nursing experience are required, with CCM/ACM preferred. This role offers a day shift, full-time schedule, and AdventHealth benefits from day one.

Qualifications

• RN licensure required
• Associates of Nursing required
• Bachelor's degree in Nursing preferred
• 2+ medical/hospital nursing experience required
• Care Management/Utilization Management experience preferred
• Leadership and communication skills required

Responsibilities

• Participates in multidisciplinary rounds to review changes in patient status, progression and discharge plans.
• Communicates with patients and families regarding emotional, social, and financial impacts of illness and mobilizes resources for identified needs.
• Assesses readmitted patients for perceived reasons for readmission.
• Organizes and facilitates patient and family care conferences with the team.
• Documents discharge planning evaluations, plans, barriers, and needs; communicates with Payors as needed.
• Assesses wholistically discharge planning needs across inpatient, observation, and ED settings to avoid delays.
• Reviews medical records to inform discharge plans.
• Develops discharge plans with contingency plans throughout hospital stay and coordinates post-acute services.
• Uses technology to communicate with post-acute services and ensure complete medical records and discharge reconciliation.
• Other duties as assigned.
