Job summary

St. Anthony North Hospital in Westminster, CO seeks an RN Care Coordinator to manage care progression and discharge planning, driving quality and cost-effective outcomes while adhering to hospital policies.

You will coordinate with utilization management, nursing, physicians, and post-acute providers to optimize care transitions and advocate for patients and families throughout the process.

Qualifications

• Graduate of an accredited school of nursing.
• Two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing in lieu of one year.
• Current RN license in the applicable state(s).
• BLS certification within 3 months of hire.

Responsibilities

• Oversee progression of care and discharge planning for identified patients.
• Coordinate with utilization management, nursing, physicians, and post-acute providers.
• Advocate for patients and families and address barriers to care transitions.
• Develop multidisciplinary discharge plans with patient involvement and postacute providers.
• Ensure timely transitions to appropriate postacute services based on clinical needs.
• Document discharge planning assessments and reassess as needed.

Education

Graduate of an accredited school of nursing

RN license in the state(s) covered (AZ/AR/CA/CO/GA/IA/KS/KY/MN/NE/NV/ND/OH/OR/TN/TX/UT/WA)

BLS certification within 3 months of hire

Bachelor's degree in Nursing or higher (BSN) or related field (preferred)

Certified Case Manager (CCM) / ACMRN / UM Certification (preferred)
