Qualifications

• California Registered Nurse License (RN) required at hire.

• CCM certification desired at hire, if available.
• 3+ years in community health nursing or case management preferred.

Responsibilities

• Establish a continuum of care plan for a patient caseload.
• Monitor quality indicators, collect data, and report outcomes.
• Liaise with patients and facilitate transitions from acute to next level of care.
