• Identify gaps or barriers in treatment plans.
• Coordinate care and referrals to outside sources.
• Ensure discharge services and medication reconciliation.
• Triage symptom-based calls per approved protocols.
• Enroll patients and manage alerts for home monitoring programs.
• Provide approved health information to callers.
• Document workflows in EPIC for tracking and quality.
• Educate patients for self-management and chronic disease education.
• Interact with care partners to discuss clinical questions and plans.
