LCMC Health is seeking a dedicated Care Manager in New Orleans, LA. You will serve as the hub of case management, coordinating patient care plans across care team members and ensuring efficient transitions and outcomes.

You will develop individualized plans, perform outreach, and guide patients through the continuum of care, bringing empathy and clinical expertise to everyday nursing practice.

Qualifications

• Current nursing license to practice in Louisiana (LSBN recognized).
• 2 years of professional nursing or care management experience.

Responsibilities

• Develop individualized care plans with patient and caregiver goals.
• Make outbound calls to assess health status and identify gaps in treatment.
• Coordinate care and referrals to external resources.
• Ensure safe discharge planning and medication reconciliation.
• Document workflows in EPIC and maintain quality tracking.
