Qualifications

• Current NYS RN license is required.
• Bachelor's degree in Nursing preferred.
• Prior nursing experience preferred; ID/DD experience is a plus.
• Strong assessment, care coordination, documentation, and communication skills.
• EMR/computer proficiency required.
• Reliable personal vehicle required.

Responsibilities

• Nursing assessments, care coordination, and health oversight for approximately 25-30 individuals.
• Travel weekly between residential homes in Brewster, Patterson, Carmel & Pawling.
• Develop Plans of Nursing Services (PONS) and maintain accurate clinical/EHR documentation.
• Oversee medications, AMAP staff, and pharmacy coordination.
• Train and provide clinical guidance to Direct Care staff.
• Coordinate medical appointments and collaborate with families, providers, and healthcare teams.
• Monitor changes in health status and coordinate follow-up or urgent care as needed.
