Job summary

Qualifications

• Associates degree in Nursing required.
• New York RN license required.
• Two years of RN experience required.
• Bachelor's degree in Nursing preferred.
• Geriatrics/managed long-term care experience preferred.

Responsibilities

• Assess, plan and coordinate care across hospital, sub-acute, long-term and home settings.
• Authorize covered services and coordinate care across payers.
• Communicate with member, family, primary care practitioners and IDT.
• Perform home visits to assess living situation and needs.
• Develop and implement Patient Centered Service Plan with IDT.
• Document care management per policy and recertification notes.
