Responsibilities

• Assess member needs and identify care gaps; assist in developing person-centered care plans.
• Monitor members' progress and adjust care plans as necessary to address changing needs.
• Coordinate access to services and care across the continuum; facilitate communication with providers and community resources.
• Provide education and support on resources, treatment adherence, disease management and self-advocacy.
• Maintain accurate documentation and ensure compliance with policies, regulations, and quality standards.
• Stay informed about current treatment trends, research, and community resources.
