• Three to five years of nursing experience.

Responsibilities

• Coordinate care and disease management across the continuum for high‑risk patients.
• Perform targeted outreach to a defined panel of high‑risk patients to ensure timely care delivery.
• Utilize registries and care team tools to manage populations.
• Conduct comprehensive clinical assessments including disease-specific needs.
• Inform patients and families about coordination of care and plans.
