Job summary

TriHealth's Center of Health Equity is seeking a Registered Nurse Care Manager for ambulatory complex care management. The role collaborates with providers and interdisciplinary teams to help patients reduce ED visits and hospital admissions through longitudinal care management.

The RN will support value-based care for primary care practices, provide patient education, and drive care coordination, compliance with NCQA standards, and advanced care planning when appropriate.

Qualifications

• Associate's Degree or diploma in Nursing (Required)
• 2-3 years experience Clinical Nursing Hospital nursing (Required)
• 2-3 years experience Clinical Nursing Care Management, Home Care (Required)
• Strong oral and written communication skills as well as strong customer service skills with phone etiquette
• Proficient computer skills
• Registered Nurse

Responsibilities

• Collaborates with the interdisciplinary team and Primary Care Providers consistently and professionally.
• Communicates with enrolled patients on a consistent basis and documents in the EMR.
• Maintains patient panels and completes outreaches with evidence-based care and NCQA standards.
• Enrolls patients in care programs, develops care plans, and conducts monthly follow-up calls.
