Qualifications

• Current state licensure as a Registered Nurse (RN).
• Basic Cardiac Life Support (BLS) certification.
• Three to five years of nursing experience.

Responsibilities

• Coordinate care and disease management across the continuum for high-risk patients.
• Perform targeted outreach to high-risk patients to ensure timely and efficient care delivery.
• Develop goals and plan interventions to maximize patient outcomes.
• Monitor progress and update plans of care as needed.
• Act as liaison between patients, families, physicians and clinical staff.
• Assist in transitions of care across different settings.
